1. Employer does not have an emergency action plan, no emergency route. 2. Employees are exposed to slip hazards around the machinery flooring due to debris and water accumulation. 3. Employer does not maintain Safety data sheets for any of the products used at the facility such as concrete products. 4. Employees are no trained on the hazards associated with the handling of wet concrete and Portland cement. 5. Blocked safety exit by the hurricane line 6. The exit doors on the north side of the plant do not have exit signs. 7. The two main Overhead cranes in the plant are having faulty brakes and are not being repaired. 8. The Conveyor system do not have safety guards installed, exposing employees to amputation hazards. 9. Employees are exposed to unsanitary working conditions, increasing the risk of illness for COVID-due to employer failure to sanitize shared tools and equipment. 10. Employees exposed to silica during wet and dry concrete and cement batch processes are nor trained in the hazards associated with silica dust. 11. Employees doing quality checks on concrete pads by breaking the pad are exposed to silica dust and not provided with respiratory protection. 12. Employer does not have a hearing conservation program for employees working inside the plant and exposed to noise levels of 92 dBA. 13. Employer allows employees to use compressed air for cleaning at 135 psig. 14. Employer does not have a permit require confined space program when employees are required to enter in concrete mixers to remove concrete residues by scraping with bricking tools. 15. Employees performing maintenance on the conveyors are exposed to electrical hazards, in that, the employer does not have a lockout/tagout program and procedures. 16.
