1) Clinical staff members, Nurses, and ER Technicians are not donning full PPE for Covid-19+ patients and PUI's being triaged and walked/wheeled trough the department. There have been occurrences of staff escorting Covid-19+ patients and PUI's from the ER triage to the ER patient room with sometimes only a mask on and without eye protection or gown. Also, the patients room door remains open while completing the ER triage assessment and while hooking up the patient to the monitors. These staff members, while headed back to their workstation, touch everything; this may include break rooms, clean and soiled utility rooms, and various other locations. 2) Nurses are seen going into Covid-19+ rooms without eye protection or gown. 3) Many staff members on the evening/midnight shift are observed not keeping their mask on or pulled up over their nose. Some staff members are observed with mask down and while have lengthy conversations with one another in patient care areas. Staff are eating and sharing food in patient care areas and while having their masks down. Clerical and Registration staff have been observed doing the same and while being in view of the ER waiting room. 4) There have been multiple staff members in the Covid-19+ rooms at the same time.Source: Osha.gov | Receipt Date: 2020-12-16
