1. The employer is not following Executive Order 20-74 which requires the facility to have a COVID-19 Preparedness Plan in place, specifically: a. The employer has not trained employees on the COVID-19 Preparedness Plan for the facility. b. Employees are not provided with personal protective equipment they need to conduct work within areas of the facility that have been quarantined due to positive COVID-19 cases. c. The employer is not isolating residents who may have or may have been exposed to COVID-19. 2. Additional safety related concerns are as follows: a. Employees working with 480V electricity are not properly trained in handling this type of equipment. b.
