- comments: observed evidence of appx. 60 or more mice droppings in lower cabinet of 2- compartment sink ,on floor next to refrigerator and on floor in between clutter on floor next to cabinets in cooking kitchen. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 9/15/20. (priority foundation 7-38-020(a)
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must repair or replace worn door gaskets on 3- door reach in cooler.
55. Physical facilities installed, maintained & clean
- comments: must remove,elevate or organize articles off of floor and away from walls in cooking kitchen.
59. Previous priority foundation violation corrected
- comments: still observed continued non compliance for violation #59- observed facility without ventilation hood system or filters installed and maintained. Must obtain, install and maintain. Priority foundation violations not corrected in time frame of previous inspection is classified as a priority violation 7-42-090. School placed on 48 hour alternative food source. No prepping, serving,or washing of any dishes. Everything must be individually pre-packaged and disposable. Instructed cps to contact cdphfood@cityofchicago. Org a request for an extension was granted until 6/30/2020 which has expired. (priority 7-42-090)
