On 8/15/21, an employee was admitted to the hospital due to contraction of COVID-19. The employee subsequently stayed overnight at the hospital and oxygen was given.

1. Employees are forced to work in close vicinity of employees that are not wearing masks. 2. High touched surfaces are are not cleaned and sanitized. 3. Bathrooms are not kept in a sanitary condition and the garbage is not removed frequently.
