Regulatory Report

Contact tracing was being done by employer's infection prevention department in collaboration with the department of health. Tracing completed on 11/16/20. Determined to be work related exposure. Writer had phoned this in to OSHA on 11/11/20 after finding out about hospitalization on 11/10/20.

Employees are not wearing masks indoors. Majority of customers are not wearing masks even though sign on door says that masks are required.
