Regulatory Report
15620 Highway 99, Lynnwood, 98037 Washington, United States
Original complaint was received by DOSH 11/09/2020. DOSH initiated a letter inquiry and phone contact shortly after. Employer provided a verbal response explaining their COVID 19 controls but did not provide a written response detailing the same. DOSH assigned an inspection on 03/25/21 to follow up on the phone fax letter inquiry. The original complaint included: Employer does not follow up properly with COVID 19 contact tracing; does not inform affected workers; remained open after recent report of exposure; does not maintain proper cleaning supplies or policies. The salon remained open with no disclosure to the public. Tropical tan has failed to screen employees for symptoms since the opening of non-essential businesses in June 2020. There is no documentation or follow up from the owners that this regulation has been enforced or followed. The owners are orchestrating deception to not only their customers but their employees as well in order to stay open. Our cleaning essentials have always run short and workers bring in their own supplies to protect from cleaning bodily fluids. Our hot water tank was broken for months, failing to properly sanitize laundry with human sweat/body fluids. 15620 HWY 99, Lynnwood WA 98037. All locations have failed to COVID 19 screen their employees. All locations do not have a COVID 19 safety protocol in place.Source: Osha.gov | Receipt Date: 2020-11-09
