1 )Tacoma General has failed to provide timely COVID -19 testing to both patients and employees, leading to an outbreak in the ICU/ACC 2) Tacoma General has failed to notify nurses of exposures to COVID-19 positive patients and employees. 3) Tacoma General nurses have reported that nurses have been allowed to continue working after a positive COVID-19 test, and while still symptomatic. 4) Tacoma General has failed to isolate COVID-19 positive patients to hot zones, risking exposure to additional patients and nurses. 5) Tacoma General has failed to complete fit testing for all nurses, in spite of their knowledge of the pandemic for approximately 11 months. 6) Tacoma General still reports a shortage of N95 masks, and limits access to nurses who wish to wear them for their own safety. 7)Tacoma General has failed to notify the proper regulating bodies about the outbreak in 6R, leading to approximately 16 positive staff (see #2, above). 8) Safety concerns at Tacoma General have increased significantly.Source: Osha.gov | Receipt Date: 2021-01-27
