1. There has recently been a COVID positive resident in the locked facility. Since then employees were supposed to be trained on how to properly wear a n95 mask and hazmat gown. Employees were also supposed to have been supplied with disposable face shields, if they didn't have safety goggles/glasses of their own to wear. The unit has not provided training on how to fit the n95 masks nor have they supplied disposable face shields. 2. Before the resident tested positive, staff were required to wear face coverings and face shields when in direct contact with patients. With the exception of one supervisor, other supervisors and the program manager have been observed on the unit where patients are served while not wearing a face shield or even a face covering at times. Staff have brought their concerns to the program manager multiple times and nothing has changed. 3. Employees were supposed to be given a weekly COVID antibody test. One employee on third shift had not been tested for 2 weeks and after completing their own test was determined to be positive. During this two week period the employee was still permitted to work and risked exposing other staff and the residents.
