Item #1: At entry points, visitors are allowed to enter the facility without being asked their COVID-19 status and/or while exhibiting symptoms. Item #2: Employees in the MICU are not provided access to additional respirators throughout their shift. Item #3: Employees in the MICU are not provided access to additional gowns throughout their shift. Item #4: The COVID-19 plan and hazard assessment did not include input from non-managerial employees and their representatives. Item #5: The employer failed to do a COVID-19 hazard assessment. Item #6: Employees in the MICU are not required to use respiratory protection and gowns when caring for patients that had not been screened or tested for SARS-Covid-2. Item #7: The employer did not provide COVID-19 training that covers all the elements required by the emergency temporary standard. Item #8: The employer failed to communicate employees of different employers the specifics of the COVID-19 plan and the COVID-19 hazards at the host location. Item #9: In the Emergency Department, MICU and COVID-19 unit located on the 3rd floor, the employer had not implemented Standard and Transmission-Based Precautions, in that, confirmed COVID-19 patients are placed in hallways, which lack adequate isolation and ventilation. Item #10: The COVID-19 unit on the third floor lacks adequate ventilation, in that, the number of air changes per hour had not been maximized.
