1. The employer did not ensure that walking and working surfaces are kept clear of objects including but not limited to, pallets, pallet jacks, plastic wrap, food items and, incomplete meal-kit boxes which present a tripping hazard to employees working in the facility. T8 CCR 3273(a). 2. The employer did not ensure that the pallets which are used to store goods on the shelving have adequate strength for the loads which are handled as well as ensure that unsafe pallets with defective parts are repaired or replaced. T8 CCR 3338(a). 3. The employer did not ensure that materials which are stacked on pallets in the warehouse were stacked in such that they didn't present a falling or collapsing hazard. T8 CCR 3241(c). 4. The employer did not ensure to implement a program to minimize Repetitive Motion injuries for employees working in the Knitting Department as well as the assembly line. T8 CCR 5110(b). 5. The employer did not ensure that operators of the forklifts in the facility have completed training as required by this section. T8 CCR 3668(a)(2). 6. The employer did not establish, implement and maintain an effective COVID-19 Prevention Plan. T8 CCR 3205(c). 7. The employer did not establish, implement and maintain an effective Injury and Illness Prevention Program (IIPP). T8 CCR 3203(a).
