IN PATIENT AREAS Patients not all wearing masks potentially exposing workers to COVID-19. Employees have tested positive for COVID-19. Patients are not always keeping at least 6 feet distance from workers including during medication administration. Barriers have not been installed in all areas. The surgical masks are very thin and disintegrate easily and employees are not permitted to change masks throughout the work shift. Not using a three-ply surgical mask. No eye protection provided Hand Sanitizer not available on all units. Housekeeping is not adequate with frequent cleaning. OUT PATIENT BUILDINGS Patients in the outpatient buildings who were under quarantine because the had been a positive patient case in the building, were allowed to go inside the gym. Ventilation system in the gym is connected to patient units creating potential for spread of COVID-19.Alleged Hazards: 10, Employees Exposed: 100
