4710 Village Square Drive, Paducah, 42001 Kentucky, United States

Dr. had COVID and worked on a Thursday. Came down on Friday (office was closed) had girls work next week without patients. The Dr. however came in 3 times that week with COVID. Girl A* got COVID (went to Baptist Hospital), Girl B* got COVID, stayed hom 2 weeks, went back to work Monday, 7/4. Girl C* has COVID in Lourdes Hospital since 6/24, put on vent. On vent 7/1 - critical pneumonia, both lungs and still on a vent today 6/7/21. Girl A* stayed home 2 weeks. Office never closed - never professionally cleaned.
