1. Emergency exit signs are not illuminated in building 13. 2. Emergency exit signs are not sufficiently posted in all areas. 3. Employees who operate powered industrial trucks have expired licenses. 4. Employees who operate scissor lifts have expired licenses. 5. Scissor lifts are being operated without being inspected. 6. Powered industrial truck operators are not being retrained after being involved in a powered industrial truck incident. 7. Powered industrial truck operators are not being recertified every three years. 8. Employees who are exposed to noise at levels above 85 decibels are not provided with annual hearing exams. 9. Employees who have been designated to utilize fire extinguishers have not received the required initial or annual training. 10. Not all overhead cranes have been marked with the appropriate load rating. 11. Compressed gas cylinders in building 13 are not stored or separated correctly. 12. Employees have not been provided blood borne pathogen training where required. 13. Employees are overexposed to plastic dust in buildings 5 and 13. 14. In building 4, there are machines that are similar to the 'former' in building 13. These machines are not properly guarded. 15. Employees have not been provided with appropriate lock out tag out training. 16. Employees who perform lock out tag out are not provided with and do not use standardized locks and/or tags.
