1. Employees are exposed to trip and fall hazards inside the kitchen and behind the bar. There are covered and uncovered floor holes inside the kitchen. The floor gives way behind the bar and employees fear falling though the floor. The employer is using mats to cover a few of the holes and soft areas of the floors. 2.
