Hazard Description: 1. Failing to furnish employees a workplace free of recognized hazards resulting in 1500 employees demonstrating symptoms of COVID-19. 2. Disinfecting, cleaning, and re-using single use N-95 respirators and other masks in violation of 1910.134 by using a respirator not in compliance with the conditions of its certification or disinfected outside of the procedures specified by 1910.134 App B-2 and procedures recommended by the respirator manufacturer. 3. Improper ventilation in certain departments that may have contributed to staff infection rates. 4.
