1. A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, quarantining employees, informing employees for possible exposure to the virus, employee returning to work, social distancing, personal protective equipment, disinfecting/sanitizing, and employee training. Twenty employees tested positive for COVID-19. Employees were not quarantined; they were forced to come to work when they were exhibiting symptoms of the virus. The employer has not allowed the employees to clean and disinfect commonly touched surfaces within the COVID-19 positive patient rooms for months. There was a shortage of face covers on January 19, 2021; and, employees had to care for patients without respiratory protection. 2. Employees, who provided care to COVID-19 patients, were not fit tested to wear a respirator.
