1. Employees are exposed to fall hazards when they are working on dryers in the wastewater area. Employees are required to get out of the scissor lifts on the dryers and in the rafters and the employer does not ensure the use of fall protection. 2. Employees are exposed to fall hazards when they are working from the scissor lift used in the wastewater area because the lift is not equipped with a back rail/self closing gate. 3. Employees are exposed to struck-by and crushed by injuries when they are operating the Skytrack vehicle and the employees have not received training on the vehicle related topics, site specific topics and evaluated prior to using the equipment. 4. Employees are exposed to struck-by injuries when operating the Skytrack vehicle and the lights on the vehicle and the parking brake switch is a screw that occasionally falls out of place and the vehicle is not inspected and removed from service until the repairs are made. 5. Employees are exposed to electrical shock when welding in the welding area because the equipment is not protected from getting wet. 6. Employees are exposed to struck-by and eye injuries when operating cut off grinders and the employer does not provide and require the use of eye and face protection. 7. Employees are exposed to illnesses including coronavirus and the employer does not require employees working in close proximity to each other including when two employees are working in the scissor lift together wear a mask covering their nose and mouth.
