1. The employer failed to maximize the quantity of outside air provided to the extent feasible through mechanical or natural ventilation. T8 CCR 3205(c)(8)(B) 2. The employer failed to give notice of the potential COVID-19 exposure, within one business day, to all employees, independent contractors, and other employees at the workplace who may have had COVID-19 exposure after an employee tested positive for COVID-19 on March 26, 2021. T8 CCR 3205(c)(3)(B)3. 3. The employer failed to install fire detection systems or fire alarms to protect employees against the hazards of fires in the workplace. Additionally, the employer failed to install carbon monoxide detectors where employees cook food. T8 CCR 3183(b)(1) & 3203(a)(4) 4. The employer failed to secure filing cabinets to prevent them from falling over. T8 CCR 3241.1(c) 5. The employer failed to ensure that all employees are separated from other persons by at least six feet, except where an employer can demonstrate that six feet of separation is not possible. Note: At fixed work locations where it is not possible to maintain the physical distancing requirement at all times, the employer shall install cleanable solid partitions that effectively reduce aerosol transmission between the employee and other persons.
