The employer failed to ensure that permanent floors in the woodshop are free of dangerous projections or obstructions, such as electrical power cords, pneumatic hoses, ventilation ducting, and projecting sharp metal objects. T8 CCR 3273(a) 2. The employer failed to provide an appropriate work platform for elevating employees using a forklift. The platform used was not attached to the boom or the base of the forks, was not provided with compliant guardrails, and did not have a slip resistant surface. T8 CCR 3657(a)&(b) 3. The employer failed to provide and ensure that face coverings are used by all employees when indoors, or when outdoors and less than six feet away from another person. T8 3205(c)(7)(A) 4. The employer failed to use a suitable dust collection system for woodworking machines. T8 CCR 4324(a) 5. The employer constructed a noise dampening box around a large compressor in the woodshop which was not designed or engineered to safely sustain all loads, stressed, and environmental conditions.
