The employer failed to ensure that permanent floors in the woodshop are free of dangerous projections or obstructions, such as electrical power cords, pneumatic hoses, ventilation ducting, and projecting sharp metal objects. The platform used was not attached to the boom or the base of the forks, was not provided with compliant guardrails, and did not have a slip resistant surface. T8 CCR 3657(a)&(b) 3.
