1. Triethylamine, chemicals 271,161 and many other materials are stored in a flammable cabinet. 2. Dry ice, handling of chemicals, and changeovers are a hazard. 3. Employees are exposed to triethylamine gas. 4. Personal protective equipment is not used on the production floor between and behind lines 1 & 2. 5. New employees who monitor chemical systems are provided subpar or no training at all. 6. An employee responsible for procedures such as lockout/tagout, and checking Baume/coating and other chemicals, had not received training. 7. MSDS have not been updated since before 2011 and are not posted or accessible to everyone (not on wall). An employee was told they could find them on the computer shared drive. The most current MSDS sheet on the computer shared drive for triethylamine was printed on 06/08/2011. 8. The employer passed a Fire Dept inspection for onsite T-gas quantities because the auditor didn't go into the room where there where quantities of the gas were over the limit. 9. contained the over the limit quantities of T-gas , so the auditor didn't look and we passed audit. 10. An employee did not receive any orientation training, safety training, or guidance when hired. 11. Many piping systems are not labeled or well identified. 12. Propane tank cabinet is not locked and sits outside. 13. The facility is not locked when no one is present allowing the potential for others to come in and have access to chemicals. 14. There is a pipe that leads to the outside (on dock/shipping side by far right entrance) with what appears to be duct tape and a slit in it. 15. Rags that are soaked in residue/muck (powder coating,oil/grime, the mix for sand cores) are disposed in what appear to be regular dumpster tubs (blue republic metal dumpster). 16. Two employees have had bloody noses during their employment. 17. Employees have noted headaches when working at the facility. 18. An employee was instructed to clean the eye wash station behind line 2 using a rag with Lime Away/Lysol. The instructions on the inside are half missing/not legible, but the legible part says the filter must be removed. 19. There are many trip and push/pull hazards for line inspectors. 20. Packaging tubs do not get cleaned thus resulting in loose powder/particles/sand being blown up toward the face when placing top layer dividers into them. 21. There are no covid practices in place for line inspectors. They are not required to wear masks and there is no social distancing. 22. It is believed that drivers don't carry the appropriate license to operate fork trucks. 23. He has been witnessed (and also a photo taken) with him 24. An employee has been observed sitting on a fork truck in idle with his phone in hand and no safety glasses on. 25. An employee with a head injury from a fall was driven home by Plant Manager instead of hospital. 26.
