1. There are live wires exposed coming from or near the 160 Grinder, exposing employees to shock hazards. 2. There are live wires exposed coming from the junction box by the bucket elevator in the pit, exposing employees to shock hazards. 3. The walking paths are not clearly painted, exposing employees to struck-by hazards from forklifts and falling product if they are outside of the walking path. 4. When super absorbent materials are grinded, the fibers go into the air, exposing employees to the health hazards of inhaling the fibers. 5. Employees are not trained on how to operate forklifts. 6. Forklifts are not inspected prior to being put into service at the start of the workday. 7. Employees must insert their entire body by crawling into the bag house vacuum and shaking the dust off the bag on a daily basis without confined space training, an attendant, or atmosphere monitoring. 8. Forklifts are operated inside the building and are accumulating exhaust vapors due to lack of ventilation. Employees are experiencing headaches as a result.
