1. The employer failed to investigate a COVID-19 case in the workplace on or about February 1, 2021. An employee who did have close contact with the COVID-19 case was not informed by the employer and was not excluded from the workplace for at least 10 days. T8 CCR 3205(c)(3)(B) 2.
