1. The employer failed to investigate a COVID-19 case in the workplace on or about February 1, 2021. An employee who did have close contact with the COVID-19 case was not informed by the employer and was not excluded from the workplace for at least 10 days. T8 CCR 3205(c)(3)(B) 2. The employer failed to clean and disinfect the areas, material, and equipment used by a COVID-19 case during the high-risk exposure period. T8 CCR 3205(c)(8)(C) 3. The employer failed to ensure that employees wear face coverings over the nose and mouth when indoors. T8 CCR 3205(c)(7)(A) 4. The employer failed to give notice of potential COVID-19 exposure within one business day to all employees who may have had COVID-19 exposure and their authorized representatives. T8 CCR 3205(c)(3)(B)3. 5. The employer failed to ensure that employees are separated from other persons by at least six feet.
