1. The employer does not enforce the use of face mask in the store and kitchen. Employees are required to wear face masks only when they are serving customers. This condition exposes employees to contracting COVID-19 in the workplace as social distancing is not practiced. At least three employees have been complaining of sore-throats. 2. The roof over the ice cream machine leaks when it rains and the water collects on the floor. This condition exposes employees to slip and fall hazard.Alleged Hazards: 2, Employees Exposed: 15
