1. The employer did not ensure to implement and maintain effective engineering and work practice controls for reducing the risk of transmitting aerosol transmissible diseases by allowing patients to expose the staff to a potential COVID-19 hazard by not enforcing facial coverings on patients. T8 CCR 5199(e)(1). 2. The employer did not ensure that employees maintain spacing of at least 6 feet while holding group therapy sessions to effectively reduce aerosol transmission between the employees. T8 CCR 5199(e)(1). 3. The employer did not ensure to implement and maintain an effective procedure for screening incoming patients or employees who work or live at the facility. T8 CCR 5199(c)(3). 4. The employer did not notify the employee(s) of any other employer may have had contact with the case or suspected case while performing activities within the scope of this section. T8 CCR 5199(h)(6)(B). 5. The employer did not provide the required training in accordance with this subsection to employees with occupational exposure to aerosol transmissible pathogens (ATP), specifically the novel pathogen SARS-CoV-2 the virus which causes COVID-19. T8 CCR 5199(i).
