The employer failed to determine, by evaluation of activities of the COVID-19 case and all locations at the workplace which may have been visited by the COVID-19 case during the high-risk exposure period, who may have had a COVID-19 exposure to a confirmed COVID-19 case. Management is not performing contact tracing. Employees are having to conduct their own contact tracing to determine if they were exposed. T8 CCR 3205(c)(3)(B)2. 2. The employer failed to give notice of the potential COVID-19 exposure, within one business day, in a way that does not reveal any personal identifying information of the COVID-19 case, to all employees who may have had COVID-19 exposure. The women's restroom, adjacent to the skilled nursing facility advice nursing station, has urine and feces on the toilet and floor and is not being cleaned regularly.
