'- No forklift training 6 ee's use during work week - Employees are walking to close to the forklift while its running - LO/TO was removed from pallet sorting machine east side of building near the office with bolt cutters from the head supervisor and was used by an employee and forklift driver placing pallets on it. The machine has been breaking down and throwing pallets and debris at employees - Pallets are stacked 36 high or more and are not secured and many have fallen and one employee was injured by a pallet - No hard hats available - Employees are not wearing masks on the floor during COVIDSource: Osha.gov | Receipt Date: 2021-02-10
