1. For on-site activities, there are no safety protocols in place and no protocols/documentation around symptoms check for those working at, visiting, or attending meetings. 2. An employee of a wholly owned portfolio company was allowed to work on-site without PPE or daily recorded symptoms check during shelter-in-place. There is no communication to employees regarding office safety measures when visiting and lack of defining what would constitute a necessary on-site activity. 3. The owner scheduled at least one known on-site meeting without any proper protocols in place. An on-site team member did not have PPE and had no knowledge of protocols. Employees visited the office without having ever received communications or guidelines around COVID office procedures. The employer was unable to provide symptoms questionnaire or address process for securing safety of employees while on-site.
