As it pertains to the employees and staff at the New Haven and Bridgeport Locations: 1. The employer is not continuously following the Centers for Disease Control and Prevention (CDC) and American Dental Association's (ADA) guidelines and recommendations for dental offices that are actively seeing patients. 2. Employees are not notified within a timely manner of a potential indirect and/or direct coronavirus (COVID-19) exposure or encounters. It is alleged that at least one (1) employee has tested positive and all employees were not notified at all and/or within a timely manner of a potential indirect and/or direct coronavirus (COVID-19) exposure or encounter 3. Employees who have prolong and/or exposure to COVID positive staff are not quarantining per the CDC and ADA guidelines. 4. After becoming away of a COVID positive staff member, proper disinfecting and cleaning is not being performed in accordance with CDC Guideline, including high touch areas at either facility. 5. CDC Guidelines, such as but not limited to, the Interim Guidance for Businesses and Employers to Plan and Respond to Coronavirus Disease 2019 (COVID-19) are not being followed. 6. Personal protective equipment (PPE) is not continuously available due to low and/or out of stock items.
