The establishment does not have a written COVID-19 Preparedness Plan in place. 2. The establishment does not have protocols in place that allow for adequate physical distancing of staff when working in shared spaces. 3. The establishment is directing employees to return to work who have been directed to isolate or quarantine. 4. The establishment does not have procedures in place to conduct health screenings of staff to identify individuals with symptoms of or potential exposure to COVID-19. 5.
