Employee at the establishment was feeling ill for four days, but was not sent home. On the fourth day of illness the employee left work mid shift because a significant other tested positive for COVID. Cleaning was not conducted after notification; except for Lysol being sprayed at employee's specified workstation. Contact tracing for employees and customers not completed. Employees were exposed to the positive employee while eating lunch, but notification was not given to all exposed employees. Shortly after exposure, one of the employees not contacted tested positive along with another employee. Work stations are not distanced properly.Alleged Hazards: 3, Employees Exposed: 25
