1. Doors on machines such as looms and tackers slide open and are kept open when the machines are operating. The doors have been plugged so that the machines can operate with the doors open. 2. Drive shafts on some of the equipment at the back side of the finish room are not covered. They should have glass windows or metal plates over them. 3. E-stop buttons are missing on some of the equipment, such as the card machine on Line 4. 4. Employees are directed to open the doors on the card machines to clean them while they are running. A brush is used and employees' arms are inside the machines while they are operating. There is no written procedure for the cleaning process. 5. The seatbelts on forklifts are plugged so that the forklifts can be operated without attaching the seatbelts. 6. Electrical panels are not locked and are accessible to unauthorized persons. 7. The employer has not provided training on lockout/tagout. 8. Cable locks have been provided to employees, but they appear to be inadequate for their intended use. 9. There is no enforcement for the wearing of face masks for COVID-19.
