1. Failure to comply with the mandate for employees to wear face coverings. The night shift employees never wear face coverings during merchandise stocking and supervisors are not enforcing the mandate. 2. Failure to comply with social distancing measures by maintaining a minimum six-foot distance between employees in public spaces. The employer is not enforcing social distancing between employees during the night shift. 3. Failure to comply with the mandate for persons/patrons to wear face coverings while in the Employer's business establishment. The employer is not enforcing the mandate requiring patrons to wear face coverings before entering the store. 4. The store is not being sanitized as required during the night shift and throughout the day to prevent the potential spread of COVID-19. 5. Employees are being retaliated against if they approach supervisors to express concerns related to safety and/or health related to the current pandemic.Alleged Hazards: 5, Employees Exposed: 50
