Regulatory Report
400 Galleria Officentre, Southfield, 48034 Michigan, United States
1. I am given poor fitting PPE to take care of patients, including COVID + patients. 2. I was fit tested for a N95, they only gave me 5 of them and now, they only have KN95 masks available to treat COVID + patients and I have not been fit tested for this mask and was told there is no need to fit test on this mask. 3. Administration told me their policy on masks wearing and use is per the CDC, not OSHA. 4. I have purchased my own NIOSH approved half mask respirator however administration is not permitting me to wear it. 5. They failed to select respirators from a sufficient number of respirator models and sizes so that the respirator is acceptable to, and correctly fits me, the user. 6. Upper management is claiming that they do not have the fit test reagent and reliable N95 masks to provide to the staff as they are "just not available at present." which is not true. 7. They have defective KN95 masks which they just keep handing them out to the staff.Source: Osha.gov | Receipt Date: 2020-12-08
