1.) Employees are only provided one face mask per week for the protection from, and prevention of, COVID-19 in the workplace. Additionally, some employees incorrectly wear their face mask only over their mouth but not over their nose. 2.) Disposable gowns are not provided to employees who are in contact with patients. 3.) Patients filling out COVID-19 questionnaires are permitted in the office even if they indicate they have had potential exposure to COVID-19. 4.) Employees have tested positive for COVID-19. Proper contact tracing was not conducted in that employees who had potential exposure to the positive employees were not required to quarantine or undergo COVID testing. 5.) Employees are not utilizing nor are they allowed to use safe needle devices.Alleged Hazards: 5, Employees Exposed: 15
