1. Employees providing close contact patient care for residents with symptoms that are consistent with COVID-19 are wearing face masks instead of the CDC recommended PPE of N95 respirator, eye protection, gown and gloves. 2. The employer has not developed a method to inform employees who provide close contact care which residents require additional PPE than a face mask. 3. The employer is not enforcing the requirement that employees complete the temperature taking and medical screening prior to entering the facility. 4. The employer is not enforcing the requirement that employees wear face masks while in the facility. 5. The employer is not frequently cleaning high contact surfaces. 6. The employer has not developed a procedure for notifying employees when they may have been exposed to COVID-19, but maintaining confidentiality as required by the Americans with Disabilities Act (ADA).Alleged Hazards: 6, Employees Exposed: 25
