3. Employees tested positive for COVID-19, however, the employer failed to deep clean and sanitize the facility as recommended by the Centers for Disease Control (CDC), thereby potentially exposing employees to the coronovirus.

4. Employees tested positive for COVID-19, however, the employer failed to immediately notify workers of the hazard, thereby potentially exposing employees to the coronovirus.
