OLYMPIC PLACE ASSISTED LIVING 20909 Olympic Place Arlington, WA 98223: Kitchen staff were not wearing masks at all. Waiters took off masks frequently when not serving. Front desk refused to pull up masks. Hazard Location: Kitchen, dining room.
